Well, after all this time I finally figured it out. We all had a clue about 10 entries back:
I did notice that I get the red light to blink once when I press any of the device buttons, with the exception of the TV button. Is this a clue?

If you put the TV setup code into the PVR device button does it work the TV? I guess that would eliminate the TV upgrade device codes as being a problem. Maybe inserting the TV upgrade code is causing some corruption elsewhere? Can you try using a built-in setup code? Then try to verify the macro ...

Well I tried rebuilding the IR code by importing all the devices again. I ended up using the 4th choice for URC6131 under the "New" file pull down to create a new file. I added all the devices and entered the macros. However, now when I upload the code to the remote, I cannot get the red light to ...

Would Toadtog, LKP, Pause, etc show up in IR if the extender was not present? Yes, but that doesn't mean it's running in the remote. The extender needs to be activated after ANY reset of the remote, which includes any upload or download done from IR.

Unless by some bizarre circumstance the TV power button didn't get mapped to the O_TV button set. Macros directly on device keys have always been tricky and perhaps the extender isn't doing them correctly. This extender has never been upgraded as far as I know, so it may have some bugs.
