At the top of this page is a link to the "File Section". Go there, and select the "Diagnosis Area". There you will see a link at the top saying "upload". Use it. Fill in the applicable sections of the template that you come to.
THEN locate your uploaded file, and post a link to it (the top thread in the diagnosis area tells you how) in this thread (otherwise, whoever might be inclined to help, might become DISinclined, as searching for a file amongst the hundreds in the Diagnosis Area is not efficient use of one's time)
I suggest not waiting too much longer, as your problem MIGHT be something an experienced user can spot quickly.
