2 buttons pressed together on universal remote?

I want to do the opposite of a macro - when left is pressed, send code 1, when 1 is pressed, send code 10, when left and 1 are pressed together, send code 11.

Is that possible on any universal remote we work with? If there is a key that works as "shift" I would need to have that shift function move to left or #1 in this example.

No it isn't possible. But many JP1 remotes can do unique functions for each of the of the following on a single button:

Short press
Long press
Double press
Shifted*
Double shifted*

*Shifted means press and release shift, then the desired button

Seems like you could most easily do codes 1, 10 and 11 with a short, long and double press of 1.

What are you trying to accomplish?
What remote?
Are you using and extender?

Can you really put an LKP and DKP on the same button? I thought they were alternatives, as together they would be two Special Functions on the same device and button.

I think that's right, as they all ultimately become keymoves, so one would overlay the other.
